End of Life Care: Compassion in the Final Chapter

Providing end-of-life care is perhaps the most sacred responsibility we hold at Springfield Care Home. Our mission is to ensure that every resident experiences a journey that is defined by dignity, peace and the absence of pain. We understand that this is an incredibly emotional time for both residents and their families, and our team is trained to provide holistic support that encompasses physical and emotional needs.

Our approach focuses on palliative excellence. We work hand-in-hand with local GPs, district nurses and palliative care specialists to manage symptoms effectively, ensuring that comfort is the absolute priority. But beyond the clinical, we focus on the human side of care. We create a tranquil environment where residents are surrounded by their own possessions and, most importantly, their loved ones. We have no restrictive visiting hours during this time, we want families to spend as much time as they need by their loved one’s side, providing them with refreshments and a private space to reflect.

Personalised care plans are vital in the final stages of life. We take the time to understand a resident’s final wishes, whether that’s a particular piece of music playing in the room, a specific religious rite or simply the presence of a pet.

We believe that a “good death” is an essential part of a “good life,” and we strive to honour the legacy of our residents by treating their final days with the utmost reverence. Supporting the family after their loss is also a key part of our care, as we offer bereavement support and a community that remembers and celebrates the life of their loved one.

Managing Parkinson’s Disease (PD), a progressive neurological condition that primarily affects movement, requires a sophisticated understanding of symptom timing, safety and physical function. For individuals navigating the tremors, rigidity and gait changes associated with Parkinson’s, a stable, familiar and predictable environment is key to minimising stress and maximising mobility. At Springfield Healthcare, we provide specialist at-home Parkinson’s care focused on delivering expert clinical oversight, co-ordinating with therapy teams and establishing routines that actively support independence and confidence in an individual’s own home.

A core challenge in managing Parkinson’s is ensuring strict adherence to medication schedules, particularly dopaminergic drugs. Even slight delays can severely impact mobility and symptom control, known as the “on-off” phenomenon. Receiving Parkinson’s care at home allows our specialist teams to precisely manage and administer medications, ensuring optimal timing to sustain an individuals “on” periods throughout the day. Routine is vital for maintaining physical function and mitigating the emotional strain caused by unpredictable symptom return. Our consistent presence also helps to manage anxiety and fatigue, which are known to exacerbate Parkinson’s symptoms.

The safety and confidence of the individual is paramount. Our complex care teams are highly trained to support the physical changes associated with PD, with a primary focus on fall prevention and promoting sustained physical activity. Key elements of our care at home support include:

  • Mobility Assistance: Providing skilled support for safe transfers and walking, particularly managing issues like “freezing of gait” (a temporary inability to move forward).

  • Therapy Integration: Working seamlessly with physiotherapists and occupational therapists to reinforce physical exercises and techniques that improve balance, flexibility and coordination.

  • Environmental Adaptation: Identifying and adjusting trip hazards, ensuring proper use of assistive devices and maintaining a home environment conducive to independent movement.

  • DBS Support: For individuals with Deep Brain Stimulation (DBS) devices, providing routine post-operative care and ensuring the stability and safety of their programming routine.

By providing these interventions at home, we reduce the need for hospitalisation due to falls and we help to maintain the individuals active participation in their recovery and management plan.

In-home Parkinson’s support focuses on the individuals overall quality of life. As the disease progresses, challenges with speech, swallowing and sleep patterns may arise. Our teams provide professional support for these changes, including mealtime assistance and techniques to aid communication.

Crucially, we also offer essential respite and guidance to family caregivers. By knowing that a specialist is managing the complex medication timings and physical risks, family members can reduce their stress and focus on providing emotional support, thereby strengthening the individual’s support network. This dedication to fostering autonomy and stability helps achieve higher levels of independence for the individual.

Springfield Healthcare provides compassionate, skilled in-home neurological care that brings stability, safety and confidence, empowering individuals with Parkinson’s to manage their symptoms and sustain their quality of life.
