We are delighted to announce that Springfield Healthcare have been chosen to deliver a brand-new care scheme under Leeds-based initiative: The Community Health and Wellbeing Pilot.
A contract has been awarded to Springfield Healthcare by Leeds City Council to fund a pilot scheme that seeks to improve home care services for vulnerable people in the area.
Alongside fellow care company Be Caring, Springfield will be taking over home care contracts from existing providers in the West Leeds area and will begin delivering the new and innovative in-home-care service from September 2024.
The Community Health and Wellbeing Pilot will:

Focus on hyper local care teams, people who live in the community that they are supporting. So, less travel for staff and reduced need for car journeys, fewer delays and more time to spend with customers.

Challenge the traditional time and task structure of home care by introducing truly flexible working, allowing them to tailor visits to the needs of the customer resulting in better outcomes for everyone.

Support three-way planning between the customer and their loved ones, the social worker and Springfield. This will ensure that needs are consistently reviewed, and the right decisions for the customer are quickly made and implemented.

Our Health and Wellbeing Care Professionals will receive specialist training to promote independence and support customers to enjoy relationships with family, friends, and the wider local community.
