Reablement is a person-centered, short-term care service designed to help individuals regain skills and confidence to live independently at home, especially after an illness, injury or hospital stay.
With reablement care, the goal is to empower individuals to relearn or find new ways to manage daily activities, such as:
- Personal care: Washing, dressing and grooming.
- Household tasks: Preparing meals and moving around the home safely.
- Community engagement: Going out and maintaining social connections.
This approach is beneficial for preventing hospital readmission and reducing the need for long-term care by helping people become self-sufficient. Reablement services are typically provided in the individual’s home by a team of professionals and can last up to six weeks, though the duration is tailored to each person’s specific goals and progress.

We offer short-term, in-home support designed to help you regain the skills and confidence to live safely and independently. We’ll work with you to practice everyday activities, from getting dressed to preparing meals, so you can continue doing the things that matter most to you.
Typically lasting up to six weeks, our reablement program is a proven way to reduce the need for long-term care and helps you stay in the comfort of your own home. With our support, you can work toward a more independent and fulfilling life.
