TDDI, or Treatment of Disease, Disorder or Injury, is the foundation of our care offerings. We are registered and clinically equipped to manage complex, ongoing medical needs under the supervision of qualified healthcare professionals.

Our robust TDDI Framework ensures that you or your loved one receives the highest standard of continuous, professional support right here at Springfield. This includes vital services such as complex medication management, wound care, specialist condition monitoring and dignified palliative support.

When you choose care with Springfield, we don’t just follow a generic plan. We collaborate with you to build a bespoke care plan, ensuring every treatment, monitoring schedule and clinical procedure is perfectly tailored to your individual diagnosis, comfort, daily routine and preferences, giving you complete peace of mind.

How to arrange complex care

We begin our service with a comprehensive, in-person assessment of your care needs. This assessment covers four key areas: your medical, functional, social and environmental requirements.

After this, we develop a personalised care plan in close collaboration with NHS teams, your family and yourself.

We ensure that these vital care plans are adjusted quickly and safely if your care needs change, reviewed regularly to align with any changing needs you may have and backed by clear documentation and robust governance processes.

At Springfield Healthcare, we embed outcome-based thinking into all our services. Our main goals are to increase your independence, improve your quality of life and reduce avoidable hospital admissions.

Your local assessment team will visit you in your home to discuss your requirements and learn about the type of carer you’re after.

If you’re happy with the assessment, we’ll help to match you with a carer who meets your preferences and has the right skills and experience for your needs.

Terry’s career in care began early, volunteering with St. John Ambulance before pursuing formal training as a Nurse in the NHS. This experience provided substantial clinical exposure across several critical areas including surgical, medical, oncology, end of life and renal care.

Currently, as Clinical Services Coordinator, Terry leverages this deep clinical expertise within the Complex Care team at Springfield, supporting the co-ordination of services, clinical skills training and assessing in homecare and live-in care.
